CRITICAL Conditions^SM is a community educational program designed by Georgia Health Decisions to help people better understand and plan for their health care at the end of life. The program features:

• The CRITICAL Conditions^SM Planning Guide a discussion tool that uses real-life scenarios to help people talk about their final health care choices with family members and record their wishes using the Georgia Advance Directive for Health Care.
